- Full name, national ID, date of birth, gender, mobile number, and email
- Club selection, board membership type, and skill level
- Sports insurance expiration date and valid insurance card image
- Player image; It is possible to select a file or take a photo with the camera.
- Respond to health self-declaration questions and register a touch signature
- For players under 18 years of age, a parent or legal guardian's signature is required.
- Optional documents: Medical certificate, federation membership number and card, relevant awards, honors, and role-related cards
- The maximum file size is 15 megabytes.
